This is for the EMS ambulance crews running non-emergency routine transports:

I'll send you a PDF copy of a form that I made as a memory aid. Not every question will be applicable to every call but it will assist you when writing your patient care report.

Training & Qualifications
I've been an EMT since 2009. I've volunteered, worked in commercial EMS, and now am employed by a hospital-based system. I'm also an EMS Instructor and I have been on a QA committee. I've attended multiple continuing education classes on the topic of writing thorough, defensible, billable patient care reports.
